The Senior Coroner, Andrew Walker, concluded that there had been a very serious failure of the hospital during the delivery of Ms Taylor’s first child not to record in her medical notes that she had a narrow birth canal and that she should ask for a caesarean section if she were to have another child. Had that been recorded, she would probably have had a caesarean section to deliver her second baby, Kristian, and he would have been born safe and well. In addition, the Coroner found that the cause of Kristian’s death was asphyxia as a consequence of prolonged and extended instrumental delivery, and that there had been a very serious failure to recognise that the instrumental delivery should have been abandoned after a fourth pull and a caesarean undertaken.
